Above are the reports from public media that cover the most recent progress of flight QZ5801 of Air Asia crashed into the Java Sea.
Based on what was reported, the stories seem to point towards that it may well be a case of Failure of Cockpit Resource Management by the pilots.
The flight was first took off “without authorized permission”;
The pilots weren’t equipped with adequate weather reports;
And, the pilots didn’t seem to use resources in the cockpit to properly fly the aircraft…
As usual, there will be denials on all the “remors ” until the final report is released by the internal aviation body.
Anyway, most aviation safety cases are due to human errors.
The bigger question is: are budget airlines rally as safe as the conventional ones? With regard to pilot training and cockpit resource management.
